I've wondered for the longest time, how were American paratroopers able to stick their grenades on their shoulders? I've tried sticking the grenade handle through my web gear, but nothing. The closest thing I got to was clipping the pin onto the hook that connects to the D-ring on the shoulder, and that eventually broke off. In Band of Brothers, many soldiers are seen carrying their grenades on their shoulders without them falling off. So, how did they do it?

They would slip the spoon of the grenade through the D-ring for the musette bag. Sometimes they would tape around the grenade and spoon, once on, to prevent them from falling off.
